* Individual Student Survey Packet *

1) Learning Styles Assessment - follow directions & transfer score to the front sheet

2) Me Poetry Activities - follow directions

* Collect Parent Signature Forms *

* Collect Folder and Supplies * 

* Permanent Folders *
Write your Last, First name on the tab.

* Goal Sheet *
Complete the Goal Sheet, and place it in your permanent folder.

* Collect Permanent Folders *
Including Completed Individual Student Survey Packet & Goal Sheet.

* Distribute Classroom Folders and Packets *
Write your first and last name, block, and Skyward Teacher on EVERY packet!

* Classroom Passes *
Write your First and Last name in INK on all 3 passes! 
Do NOT lose them.

* Literature Book Activity *
Due at the end of class.

* Cool-Down *
What needs to be here by Tuesday?
What will YOU have to do if you do not have your signature form here by Tuesday?
Place packets and any other papers in your classroom folder.
Pick-up space and push in chairs. (4th block - stack chairs on tables.

Don't forget to place your classroom folder in the appropriate bin.
